APPOINTMENT SCHEDULING
- To schedule an appointment as a new client, we do require a $106 deposit for each pet. Please note if you need to change or cancel this appointment the deposit IS refundable
up to 48 business hours prior to the scheduled appointment.
- For appointments with two (2) or more pets, we require a $106 deposit for each pet to schedule.
- All surgical and dental procedures require a $250 at the time of scheduling to hold the appointment.
- All specialist appointments require a $400 deposit at the time of scheduling to hold the appointment.
*All deposits will be applied toward the exam/appointment fee at the time of your pet(s) visit.
CANCELLATION POLICY
Our goal is to provide quality medical care in a timely manner. No-shows, late shows and cancellations inconvenience those individuals who need access to urgent veterinary care. Therefore, we would like to make you aware of our policy regarding missed appointments and late cancellations.
- A MISSED appointment or a “no-show” is when a client fails to show up for an appointment without a phone call or email.
- We allow a 10-minute grace period for doctor’s appointments. If you are more than 10 minutes late for your appointment, this will be considered a missed appointment. We will ask you to reschedule to allow us to spend the appropriate time to answer your questions to best help your pet.
- You may be subject to a cancellation fee if you miss or cancel your appointment within 48 business hours of the scheduled appointment.
- Repeat no-shows or last-minute cancellations may require that we obtain a non-refundable deposit to hold future doctor appointments.
- Specialist Deposits are non-refundable if appointments are missed or cancelled within 48 business hours
of the scheduled appointment.
- Surgical / Dental Deposits are non-refundable if the appointment is missed or cancelled within 1 week
of the scheduled appointment.
MUTUAL RESPECT POLICY
Here are Loyal Family Veterinary Hospital, we are committed to providing the highest quality care for your beloved pets. Our dedicated team of veterinary professionals take pride in their work, and we aim to create a safe and respectful environment for both our clients and our staff. In order to maintain this standard of care, we have established a policy that outlines the mutual respect necessary to operate effectively.
The veterinary field has one of the highest suicide and burnout rates among professions, at Loyal Family Veterinary Hospital, we must prioritize the mental well-being of all our team members. To support mental and physical safety, enabling us to operate at our peak for you and your pets, we require all clients to treat our entire team with mutual respect and compassion, just as we treat you and your pets. The following behaviors will not be tolerated:
- Verbal or physical abuse
- Disruptive behavior
- Profanity or disrespect
- Intimidation tactics or making threats
- Emotional blackmail
- Allowing your pet to intimidate or threaten another person or pet
- Discriminatory comments or actions
- Failure to comply with the requests of our staff, including leashing/restraining your pet
- Any form of harassment
Failure to comply with the mutual respect policy may result in the termination of your relationship with Loyal Family Veterinary Hospital.
CONSENT TO TREATMENT
I, the undersigned, am the owner or agent for the owner of the animal described above. I have the full and exclusive authority to execute this consent and am over 18 years of age.
- I give permission to doctors, staff, authorized agents, or representatives of this hospital to examine, prescribe for, and treat my pets.
- I agree to pay for all services rendered and medications, goods, and supplies when purchased.
- I understand that all fees are due at the time services are rendered and the hospital accepts all major credit cards. No cash or checks will be accepted.
- I understand that a deposit will be required for surgical/dental, specialist appointment and may be collected for other medical diagnostics or treatments.
- I understand that Loyal Family does not offer overnight hospitalization care and if my pet ever requires overnight hospitalization, I will be responsible for providing transportation to an overnight veterinary hospital.
