What are the requirements?

· Offer is good for new and existing clients.

· New clients require an initial exam prior to scheduling surgery.

· Existing clients must have a current/up to date Annual Exam on file.

· All patients must have completed pre-op bloodwork within 3 months of the scheduled surgery appointment.

What are the exemptions?
